Allow for multiple effective dates per rate table with historical view

The ability to have more than one effective date per rate table - at the moment rate tables in Viewpoint only allows for two rates and one effective date. This has many limitations especially when you have more than one rate update per annum. It will also be good to view historical rates per effective date for a rate table. It will beneficial from an auditing point of view and to make historical adjustments knowing the correct rate will be used. This needs to be applied to all rate tables in Viewpoint.
