Add company name and address to header of certified payroll report

Currently, the certified report has the company name in the report footer only.  On many occassions we have been asked to add this information by hand.  Additionally, the contract customer name from the JC Contract Master could be shown as well.

  Yes! Nobody who gets our report knows who it is from. Could get lost in piles of papers on someones desk
