The Oregon Department of Revenue (DOR) released a new state-specific W-4 form in 2019 which must be completed by all new employees (at least at our firm). This new checkbox would indicate the new employee has completed and provided the state-specific W-4 form. The existing field could be renamed to "Federal W-4 Completed". Having two check boxes would provide HR more complete and accurate information with regard to W-4 forms.
