Need to have additional fields added in Vista to capture additional provider information for Medical (medical group, doctor, PCP number) and Dental (doctor, group number) HMO benefits for all dependents. Also, a checkbox field should be added if the employee wants to request a provider auto-assignment instead. Any data entered by the employee is stored in Vista and used in future enrollment periods. All of these fields should be surfaced in the HRM Employee Portal to be filled out in the Benefits workflow. There is currently no way to capture this information specifically and the only workaround is to use a notes field.
